Donor Campaign Order Form

This file is in Excel format.  Please fill in the boxes in the form for each donor, and include your job name, contact info, which donor phase set # you are currently in, and email.  There are tabs for 4x8, 5.5x8, and 8x8 donor recognition sizes.
